Vet visit checklist for a diabetic dog or cat

A diabetes check goes further when you arrive with the record in hand and your questions written down. Tick off what to bring, take the questions in with you, and note the answers before you leave.

Use your browser's print option to keep a paper copy for the fridge or the consulting room.

Bring with you

  • Your record of insulin given: the times, and the amount your vet has told you to give
  • Your glucose readings, or the notes and printout from your monitor, with dates and times
  • A note of meals eaten, meals refused and treats given
  • Anything you have noticed about drinking, toilet trips, energy, weight or coat
  • The insulin pen or vial and the syringes you use, so your vet can see exactly what is in use
  • The names of any other medicines or supplements
  • The food and treats your pet is eating now, or a photo of the labels
  • Your pet insurance policy number, if you plan to claim

Questions to ask your vet

Your vet knows your pet, so the answers are theirs to give. Write them in the space beside each question.

  • What glucose range are we aiming for with my pet?
  • How often should I test at home, and at what times of day?
  • What should I do if I miss a dose, or am not sure it went in?
  • Which signs mean I should ring you the same day, and which mean an emergency?
  • Who do I ring out of hours, and what is the number?
  • Is the current food right for my pet, and how should I handle treats?
  • When is the next check, and will it include a glucose curve?
  • What will the next round of tests, insulin and supplies cost, and can I have a written prescription?
  • Who else looks after my pet, and what do they need to be told?

Before you leave

  • Write down any change to the plan, in your vet’s own words
  • Check you know the next appointment date and when to reorder insulin
  • Add the changes to your record straight away, while they are fresh
  • Tell anyone else who gives insulin about the changes
