Daily log sheet for a diabetic dog or cat

A paper record of insulin, meals and readings, with a column for who wrote each entry. It is blank on purpose. Fill in the amounts and times your vet has set, and take the sheet to your next appointment.

Use your browser's print option. The sheet prints in landscape, one week per page.

How to use the sheet

  • Write the entry straight after you do the task, not at the end of the day, so it is right when someone else picks it up.
  • Copy the insulin amount and times from what your vet told you. If you are unsure, ask your vet.
  • Use the last column so each person's entries can be told apart. It matters most when two or more people share the care.
  • Put anything you want to raise at the next visit in the notes column: appetite, drinking, toilet trips, energy, or a meal that was refused.
  • Keep the pages together and bring them to appointments.

Not sure what to ask at the next visit? Use the vet visit checklist. If other people help with care, fill in the household and carer handover sheet as well.
